Ensuring the wellness and safety of your children is a top priority at St. Anthony de Padua Catholic School. We are committed to providing a safe and secure campus where students can live out their faith, grow to love learning, and thrive as happy, healthy kids.
The School Wellness Ministry Program provides health resources for the Catholic schools in our area. This ministry helps our school community stay healthy and meet state requirements for students.
All student health forms should be completed upon enrollment and updated before the start of each school year. If your child must have medication administered, medication forms should be completed before the start of the school year.
Immunization records must also remain up to date at the school. Please complete a form when new immunizations are administered at your child's checkup.
